How to access the NDSS subsidy

There are two major categories of subsidy, fully subsidised and co-payment. Use this information to find out which category you may be eligible for.

To be eligible for access to NDSS subsidy under this eligibility group, a child or young person must meet the criteria in one of the following categories:

Category A – for children 10 years or younger

Children 10 years of age and younger with type 1 diabetes will be eligible for NDSS subsidy if they meet ALL of the following criteria:

  • the child is expected to benefit clinically from the use of FreeStyle Libre/CGM; and
  • the family/carer has the willingness and capability to use FreeStyle Libre/CGM; and
  • the family/carer has the commitment to actively participate in a diabetes management plan that incorporates FreeStyle Libre/CGM.

NOTE: A child who has been accessing FreeStyle Libre/CGM products through the initiative will continue to have subsidised access after they turn 11. They will not need to be re-assessed under Category B.

Category B – for children and young people aged 11 to less than 21 years

Children and young people with type 1 diabetes aged from 11 years to less than 21 years will be eligible for NDSS subsidy if they meet ALL of the following criteria:

  • the person is expected to benefit clinically from the use of FreeStyle Libre/CGM; and
  • the person or family/carer has the willingness and capability to use FreeStyle Libre/CGM; and
  • the person or family/carer has the commitment to actively participate in a diabetes management plan which incorporates FreeStyle Libre/CGM.

AND they meet ONE OR MORE of the following criteria:

  • frequent significant hypoglycaemia—more than one episode a year of significant hypoglycaemia requiring external, third-party assistance; and/or
  • impaired awareness of hypoglycaemia; and/or
  • inability to recognise, or communicate about, symptoms of hypoglycaemia; and/or
  • significant fear of hypoglycaemia for the child/young person or a family member/carer, which is seriously affecting the health and wellbeing of the child or young person or contributing to hyperglycaemia as a reaction to this fear.

People with type 1 diabetes aged 21 years and older who have valid concessional status

To be eligible for access to NDSS subsidy under this eligibility group, the person must have type 1 diabetes, aged 21 years or older and meet ALL of the following criteria:

  • the person is expected to benefit clinically from the use of FreeStyle Libre/CGM; and
  • the person or family/carer has the willingness and capability to use FreeStyle Libre/CGM; and
  • the person or family/carer has the commitment to actively participate in a diabetes management plan which incorporates FreeStyle Libre/CGM.

In addition, the person must have valid concession status. 

Concessional Status

The required concessional status for this eligibility group is consistent with existing NDSS concessional arrangements but excludes the Safety Net Concession Card (SNCC) and the Safety Net Entitlement Card (SNEC) categories. People with the following valid concessional types will be eligible for the initiative (if they also meet the other eligibility criteria):

  • Commonwealth Seniors Health Card (as issued by DHS* or DVA**)
  • Commonwealth Pensioner Concession Card (as issued by DHS or DVA)
  • Commonwealth Health Care Card (as issued by DHS or DVA)
  • DVA Gold Card
  • DVA White Card
  • Identifies as Aboriginal and/or Torres Strait Islander person

What if I am already subsidised on another CGM device and wish to upgrade to FreeStyle Libre 2?:

If you wish to upgrade to FreeStyle Libre 2, your authorised healthcare professional will need submit a switch form on your behalf.

To be eligible for access to NDSS subsidy under this eligibility group, the person must meet ALL of the following criteria:

  • the person is expected to benefit clinically from the use of FreeStyle Libre/CGM; and
  • the person or family/carer has the willingness and capability to use FreeStyle Libre/CGM; and
  • the person or family/carer has the commitment to actively participate in a diabetes management plan which incorporates FreeStyle Libre/CGM.

In addition, the person must meet the criteria in one of the following categories:

Catergory A - Pre-pregnancy

The person with type 1 diabetes is considered as actively planning pregnancy and should be having regular engagement with pre-conception care services such as an endocrinologist, diabetologist and/or specialist physician, ideally at least every 6-8 weeks and more frequently if there is sub-optimal glycaemic control. An authorised health professional may certify eligibility for an initial 6-month period on the basis of the person seeking active pre-pregnancy care and committing to regular engagement with the pre-conception care service provider.

If at the end of the initial 6-month period the pre-conception care is continuing, an authorised health professional may certify eligibility for a further 6-month period. A maximum of two, 6-month access periods (up to a maximum period of 12 months) may be authorised.

After an initial 12 months eligibility—supporting documentation is required for an extension of access. 

There may be exceptional circumstances for an extension of this timeframe that will be considered on a case-by-case basis, noting these will be:

  • considered by an appropriately constituted expert panel established by Diabetes Australia that will provide a recommendation to the Department of Health; and
  • the Department of Health will be the deciding body for all of these cases.

Any extension to the pre-pregnancy period will be considered on a case-by-case basis and require documented evidence of exceptional circumstances—such as that the person has been referred to fertility services for assessment. Your application for an extension of access to FreeStyle Libre/CGM products after the initial 12 months requires supporting documentation (on letterhead):

  • from both your diabetes health professional (authorised to certify FreeStyle Libre/CGM applications) and your fertility specialist, obstetrician / gynaecologist, confirming that you are continuing to plan pregnancy;
  • with your first name, last name and date of birth;
  • with the date you were referred to or first attended fertility specialist services;
  • including confirmation of your ongoing fertility assessment or management.

Your applications for an extension of access to FreeStyle Libre/CGM products for a further six-month period, are considered on a case-by-case basis. At the end of this six-month period, further six-month extensions may be granted subject to the same documentation being provided.

Category B – Pregnant/Post-Pregnancy

The person with type 1 diabetes has a confirmed pregnancy and is regularly engaging with a health professional such as an endocrinologist, diabetologist and/or specialist physician. An authorised health professional may certify eligibility that will continue until 3 months after the expected date of birth of the baby.

To be eligible for NDSS under this eligibility group, the child or young person must have been diagnosed with one of the conditions listed in this table and meet the criteria in one of the following categories:

Category A – for children 10 years or younger

Children 10 years of age and younger who have been diagnosed with one of the conditions listed in this table will be eligible for NDSS subsidy if they meet ALL of the following criteria:

  • the child is expected to benefit clinically from the use of FreeStyle Libre/CGM; and
  • the family/carer has the willingness and capability to use FreeStyle Libre/CGM; and
  • the family/carer has the commitment to actively participate in a diabetes management plan that incorporates FreeStyle Libre/CGM.

NOTE: A child who has been accessing FreeStyle Libre/CGM products through the initiative will continue to have subsidised access after they turn 11. They will not need to be re-assessed under Category B.

Category B – for children and young people aged 11 to less than 21 years

Children and young people who have been diagnosed with one of the conditions listed in this table will be eligible for NDSS subsidy if they meet ALL of the following criteria:

  • the person is expected to benefit clinically from the use of FreeStyle Libre/CGM; and
  • the person or family/carer has the willingness and capability to use FreeStyle Libre/CGM; and
  • the person or family/carer has the commitment to actively participate in a diabetes management plan which incorporates FreeStyle Libre/CGM.

AND they meet ONE OR MORE of the following criteria:

Expanded type 1 Subsidy Available From 1 July 2022

Co-Payment subsidy

The co-payment subsidy is available to those who are not eligible for a full NDSS subsidy. Once you receive subsidised access, each FreeStyle Libre 2 sensor will cost you $15 per box. We recommend ordering at least 2 boxes of sensors at a time to give you a month’s supply.

How much is the co-payment for FreeStyle Libre sensors?:

The co-payment is $15 per single box of the FreeStyle Libre 2 sensor, equaling $30 for two boxes of FreeStyle Libre 2 sensors for up to 28 days of wear.

From 1 July 2022, access to FreeStyle Libre 2 has been subsidised for all Australians living with type 1 diabetes. The information on this page will help you assess your eligibility for the subsidy, how to access it and what to expect from the application process. We also answer your frequently asked questions.

NDSS Subsidy Application Process:

An authorised healthcare professional will assess your eligibility, complete and submit the NDSS form for NDSS approval.

Handy tip: During the Coronavirus (COVID-19) pandemic, the NDSS has announced people living with diabetes no longer need to sign the eligibility forms required to access Freestyle Libre under the NDSS subsidy. Authorised healthcare professionals are now able to complete the form for you, without the need for you to co-sign.

Once the NDSS confirms your eligibility, you will be directly informed by the NDSS.

Once the NDSS confirms your eligibility, you will need a device to scan your sensor.

Once approved as an eligible registrant, you will be given instructions on how to access a dedicated website in order to:

  • Download the free of charge FreeStyle LibreLink app onto your smartphone to scan your sensor and/or;
  • Order a FreeStyle Libre 2 reader* to scan your sensor
  • Access support and instructions on how to get started with FreeStyle Libre 2

Frequently Asked Questions

I am over 21, have type 1 diabetes and have never used FreeStyle Libre 2 before but want to start using subsidised FreeStyle Libre 2

You will need to first check the type of subsidy access you qualify for on SubsidyChecker.com.au and will need to see an authorised healthcare professional to complete a Continuous and Flash Glucose Monitoring Access Form.

I am over 21, have type 1 diabetes and have previously used another CGM device within the date range specified by the NDSS for the streamlined pathway (1 January 2022 – 1 June 2022) BUT want to start using subsidised FreeStyle Libre 2

You will need to see an authorised diabetes healthcare professional and they will need to complete a Continuous and Flash Glucose Monitoring Access Form.

How much would I need to pay for FreeStyle Libre 2 sensors under co-payment?

The co-payment per box of the FreeStyle Libre 2 sensor for up to 14 days of wear, is $15 or $30 for two boxes.

Why are the manufacturers sensor co-payment prices different on the NDSS subsidy?

The co-payment you will pay is the equivalent of $32.50 per one months supply. The NDSS subsidy will be applied to the equivalent of one months supply based on the manufacturer’s recommended sensor lifespan. The co-payment amount will also be influenced by how many sensors the manufacturer requires you to purchase at a time when you order. FreeStyle Libre 2 sensors can be worn up to 14 days.

I’m currently accessing another subsidised CGM through the NDSS but wish to upgrade to FreeStyle Libre 2, how do I do this?

If you wish to upgrade to FreeStyle Libre 2, your authorised healthcare professional will need submit a switch form on your behalf.

I would like to use the FreeStyle Libre 2 reader with my FreeStyle Libre 2 sensor. Is there a co-payment and how do I get one?

We recommend using the FreeStyle Libre 2 sensor with the FreeStyle LibreLink app (free to download) - it enables the option of sharing glucose data and alarms in real-time with designated family and friends via the LibreLinkUp app. Users can also experience the ease of sharing of glucose reports and data with healthcare professionals via LibreView.

If you are unable to use the FreeStyle LibreLink app, eligible users can request a complimentary FreeStyle Libre 2 reader at www.scanmysensor.com.au and follow the instructions on the page.

I currently have an active subscription account with you. Will I be able to access the subsidised sensors through my ongoing subscription?

The subsidised access to FreeStyle Libre 2 sensors is administered by the NDSS. To check eligibility and information on how to access the NDSS subsidy, visit www.SubsidyChecker.com.au

You will need to cancel your active subscription by logging into your account on www.FreeStyleLibre.com.au once you received access to subsidised FreeStyle Libre 2 sensors which are only available via NDSS access points/pharmacies.

How do I know if I have been approved for the NDSS subsidy after completing the streamlined pathway form?

Once you complete the streamlined pathway form, you will receive a notification in the coming days from the NDSS confirming that your information has been validated with subsidy commencement.

I have Type 2 diabetes and am on insulin. Do I qualify for the newly expanded NDSS subsidy?

The expanded subsidy is for people living with type 1 diabetes. We are committed to continuing to work with the Government to bring the subsidy to all insulin-requiring people with diabetes.

What does concessional mean? What cards are eligible?

People with the following valid concessional types will be eligible for the initiative (if they also meet the other eligibility criteria):

Will my NDSS access point stock FreeStyle Libre 2 sensors or will they need to be ordered in?

Once approval is granted from the NDSS, we suggest you get in touch with your preferred NDSS access point / pharmacy as soon as possible to discuss supply arrangements. If the NDSS access point / pharmacy does not have FreeStyle Libre 2 sensors in stock they will be able to order it, and have it delivered. Please consult with your local access point / pharmacy for more information.

Now that I am on FreeStyle Libre 2, can I still order test strips through the NDSS?

After receiving NDSS approval for FreeStyle Libre 2, current access to blood glucose test strips will remain unchanged. If you are using the FreeStyle Libre 2 reader to scan your sensor, you can also use FreeStyle Optium Blood Glucose Test Strips to measure your blood glucose and FreeStyle Optium Blood ß-Ketone Test Strips to measure your blood ketones using the reader.

Which ‘authorised healthcare professional’ can sign and submit my eligibility form?

The NDSS lists Endocrinologists/Diabetologists, Credentialled Diabetes Educators, or physicians or nurse practitioners who specialise in diabetes as authorised healthcare professionals.
